What's Included on a Superbill?
A superbill typically contains all the information necessary to submit to your health insurance for reimbursement. Specifically it may include:
- Provider name and credentials
- NPI
- Tax ID
- Practice Address
- Client Name and DOB
- ICD-10 Diagnosis
- CPT Code
- Date of Service
- Fee
- Amount Paid
Therapy notes are not included on the superbill.
What is a Superbill and How Do You Use One?
Therapy professionals who do not take insurance are often able to provide something called a “superbill.” This document serves as a record of your sessions and the services rendered. It includes the provider’s ID, the patient’s diagnosis, and the services rendered. It also includes the cost, amount paid, and the patient’s name and address. You may submit the superbill to your insurance provider to request reimbursement for the expense.
Does Having a Superbill Guarantee Reimbursement?
The short answer is no. Ultimately, reimbursement is decided by your insurance provider. If your provider requests additional information to be added to your superbill, please let your therapist know and we will do our best to add that information.
